Customer Acknowledgment and Signature
I hereby acknowledge that all the aforementioned information are valid and conform to reality and I bear the full liability for any invalid information. Moreover, I declare that the subject of the complaint has not been considered before the Court and I commit not to take any other action in the case of agreeing with the Company upon a corrective measure for the said complaint with the commitment of the company to the agreed resolution. In addition, I commit not to resubmit any complaint to the Central Bank of Kuwait concerning the same subject.
Complaints should be delivered in one of the following methods:
- 1- By hand to the Complaint Unit in the Company
- 2- By mail to the attention of the Head of Complaint Unit
- 3- By email to the head of complain unit using the above form
The complaint made by the customer will be replied to within 30 working days from the date of receiving it by the Complaint Unit.
